Removing a trophy installation (by simply removing the folder for this installation) doesn't updates the "index", and it displays a "corrupt icon" in XMB.

Two ways to fix this "corrupt icon" are by repeating the installation (the installation will fail, but the icon is removed), or from "recovery mode" menu by the option "rebuild database"
